Parents, are you confused as to what forms to fill out each time you go on a Field Trip?  Have you ever told the Teacher that you have already filled out that form?  Well, this page was made to answer all those questions

and we have also added the forms here!

ALL VOLUNTEERS

All Volunteers at Marvista School must fill out a VOLUNTEER INFORMATION FORM with a copy of your Drivers License that is turned into Washington State Patrol.  Once cleared, you will be approved to volunteer at Marvista for two years. You may ask your teacher or call the office to find out if you are cleared.  We only need a copy of your Driver's License ONCE.  Once you have submitted a copy of your License, we will not need another copy until the next school year.

Paperwork Needed:

VOLUNTEER INFORMATION FORM - Expires every two years.
FIELD TRIP CONSENT FORM - Permission Slip

COPY OF DRIVER'S LICENSE - Only once and then shredded at the end of the school year.

FIELD TRIPS with PARENT DRIVERS:

If you are Volunteering to DRIVE on a Field Trip, you must fill out a VOLUNTEER DRIVER CHECKLIST FORM everytime you drive on a Field Trip.  This form will then be signed by Mrs. Kim for approval and then to SECURITY.  Security, must receive this form 2 days before the date of the Field Trip. 

Paperwork Needed:

VOLUNTEER DRIVER CHECKLIST FORM - Form needed EVERY TIME YOU DRIVE ON A FIELD TRIP!
FIELD TRIP CONSENT FORM - Permission Form.

COPY OF DRIVERS LICENSE - Only once and then shredded at the end of school year.

COPY OF INSURANCE - Only once and then shredded at the end of the school year.

FIELD TRIPS taking the SCHOOL BUS:

This field trip does not require Parent Drivers so you do not need to fill out the VOLUNTEER DRIVER CHECKLIST.  You would follow the steps as you would for a Parent Volunteer.
